TUNKHANNOCK AMBULANCE’SMEMBERSHIP DRIVE
Dear current and prospective members,
As you may know, memberships in TCAA expire each year on June 30th. With that date approaching, we have begin our membership drive. Memberships are open to residents and businesses in the communities of Eaton, Lemon, and Tunkhannock Townships and Tunkhannock Borough. This mailing contains an application card and return envelope, which we urge you to complete and mail to us at your earliest convenience, to ensure uninterrupted coverage (current members) or to establish coverage (new members).
Tunkhannock Ambulance is a non-profit, all volunteer, community support organization, licensed by the Commonwealth of Pennsylvania and responsible for: emergency medical care and transportation for the sick and injured to the nearest appropriate medical facility; emergency road/vehicle rescue; water (dive) rescue; emergency transfers from Tyler Memorial to other area hospitals; and search and rescue within the above communities -- 24 hours a day. In addition, we may provide (with personnel availability/operational considerations permitting) non-emergency transportation for our members whose conditions make travel by ambulance medically necessary. This could include transportation from a local hospital to the patient’s residence or from one area hospital to another, where the patient is to be admitted.
When ambulance services are required, the patient is responsible for all charges; however, your TCAA membership will forgive balances/co-payments not covered by your insurance. Your signature on the enclosed Membership Application Card authorizes TCAA to receive payment from your insurance carrier. These memberships, coupled with our billings and donations, constitute our only sources of revenue, through which we support, maintain, and operate this vital community resource.

TCAA MEMBERSHIP INFORMATION: PLEASE RETAIN FOR FUTURE REFERENCE
1. This mailing is intended for residents and businesses within the communities of Eaton, Lemon, and Tunkhannock Townships and Tunkhannock Borough only. If you reside elsewhere and received this correspondence, we apologize for any inconvenience.
2. The TCAA Membership begins on July 1st and expires on June 30th ; however, applications received during the Membership Drive are considered to be effective upon receipt.
3. TCAA is not affiliated with any Advanced Life Support/Mobile Intensive Care Units (ALS/MICUs); billings for such services are made by their company/sponsoring hospital and are separate from any TCAA billing.
4. Non-Emergency transportation under conditions of “medical necessity” (for example, from Tyler to a patient’s home of record within the TCAA Service Area after release from the hospital) requires a valid statement of need for the use of an ambulance (i.e., Physician’s Certification Statement). Because of limited numbers of volunteers in general, but particularly during daytime hours on weekdays, and the requirement to be able to respond to emergency calls, non-emergency transports will normally be scheduled between 6PM and 10PM weekdays and between 1 PM and 5PM on weekends. Note: As stated above, our current non-emergency capability is limited; thus, while it remains our goal to provide these services, there may be instances where we will be unable to do so due to personnel limitations and/or operational constraints.
5. For similar reasons, we do not transport patients to doctors’ offices, clinics, or hospitals for routine appointments, tests, or treatments.
6. Federal law (the Health Insurance Portability and Accountability Act of 1996) requires us to maintain the privacy of certain confidential health care data, known as Protected Health Information (PHI), and to provide you at the time of ambulance service, or upon request, a notice of our legal duties and privacy practices with respect to your PHI.
